Successful alectinib treatment after crizotinib-induced interstitial lung disease

上週聽了ALK-inhibitor toxicity management的演講，今天碰巧在Respirology Case Report看到這個用Alectinib after crizotinib-induced ILD的case report.

70-year-old female, non-smoker, diagnosed with SqCC of lung打過Cis/Gem後來rebiopsy後改成adenocarcinoma with ALK re-arrangement。

用Crizotinib兩個月後發生ILD，用methylprednisolone 1g QD x 3days再改成prednisolone 40mg QD然後花10週taper到off. 之後改打Carbo/Pem/Beva再用mPem/Beva，PD之後換成Alectinib，用了十個月沒有發生ILD。

心得: 

1. Female, non-smoker 一開始診斷SqCC應該會有很多疑慮?
2. 用MTP 1g QD打三天再換成口服慢慢taper蠻像以前敝院某位醫師的practice，目前經驗到的也是這樣子。
3. Alectinib在台灣不知道何時會上市..
